In this community-based Social Work role, you will be supporting older people with complex health needs to maximise their independence and to live their lives fully in their community.

You will be working alongside and adding your value to a highly professional and experienced Multi-Disciplinary Team that strives for excellence and does so with a heart for the people we serve and each other.

You will be encouraged to bring a whanaungatanga and strengths-based lens to the Enliven team.

This role includes:

  • Case management and assessment
  • Clear and concise documentation
  • Liaison with clients, client whanau/family, other health professionals and services.
  • Building and maintaining positive relationships both internally and externally.

To be successful in this role you'll be a qualified and Registered Social Worker with experience working within the health sector or with complex health and disability needs. You'll also:

  • Have exceptional time management skills and excellent documentation and communication skills.
  • Have highly developed clinical skills
  • Be a team player and be reliable.
  • Bring professional integrity
  • Have energy and passion for the role
  • Hold a full clean NZ drivers license.
